Entry Form Definition - What Is a Sweepstakes Entry Form?

By , About.com Guide

Definition:
An entry form is the method that online sweepstakes' sponsors use to collect the information they need to enter people into the sweepstakes. Each individual piece of information, such as First Name or Street Address, can be entered into a separate field. This makes it easy for sponsors to be able to sort and manage the information they receive.

Fields on a sweepstakes entry form can be optional or required. Required fields must be filled out or the entry form won't be accepted; even if it it appears to go through, the entrant may be disqualified. Optional fields can be skipped without penalty.

Most entry forms differentiate optional fields from required fields by using an asterisk (*). Somewhere on the form, it will state whether an asterisk denotes a required or optional field.

Sweepstakes entry forms may also include opt in boxes, which let entrants request additional information from the sponsoring company. For more information, see Show I Opt In for Sweepstakes Newsletters?
